The Opportunity:

As a SEND Transition Advisor, you will play a vital role in supporting students with disabilities as they transition into further education and beyond. This is a unique opportunity to make a real difference in the lives of students and help them achieve their full potential.

Key Responsibilities:

  • Coordinate and deliver school engagement, personalised transition planning, and support for students with EHCPs
  • Contribute to the EHCP consultation process and ensure student needs are fully understood and addressed
  • Act as a key point of contact for students, families, and stakeholders prior to starting college
  • Manage a caseload of EHCP students and provide ongoing support throughout their journey
  • Collaborate with the SEND and Quality Teams to identify and address staff training needs
  • Provide line management and support to Specialist Learning Support Assistants
  • Ensure all transition activities align with the SEND Code of Practice and further education standards

Qualifications & Skills:

Required

  • In-depth knowledge of the SEND Code of Practice and barriers to learning
  • Experience in further education and careers guidance
  • Strong organisational and communication skills
  • Ability to work collaboratively with students, families, and stakeholders
  • Experience in managing a caseload and providing individualised support
  • Experience in transition planning and coordination

Preferred

  • Experience in EHCP consultation and support
  • Experience in managing a team of Specialist Learning Support Assistants
  • Experience in staff training and development
  • Experience in working with students with a wide range of needs and backgrounds
